# Application Form Design

# SECTION 4: APPLICATION FORM DESIGN

Application forms are designed to evaluate thinking patterns, leadership ability, and alignment with role expectations.

### Key Principles

* **Conciseness**: Forms must remain concise while producing high signal responses.
* **Automation**: Basic candidate information such as name and email is collected automatically by the system and should not be requested again.
* **Length**: Application forms typically contain between six and eight questions.

### Question Types Used

1. **Short Text**: Used for concise reasoning or philosophy questions.
2. **Long Text**: Used for structured explanation and decision making scenarios.
3. **Yes or No**: Used to identify experience or exposure.
4. **Dropdown or Multiple Choice**: Used to identify behavioral tendencies or leadership orientation.
5. **File Upload**: Used when the role requires demonstration such as presentations or video introductions.
